There are two sets of codes for psychotherapy. One set of rules is used to notify only psychotherapy: CPT 90832- 30 minutes, CPT 90834-45 minutes, and CPT 90837-60 minutes. While social services, psychologists, and therapists are the most likely to report these, psychiatrists, psychiatric NPs, and PAs can also if medication management is not conducted during the same visit. These are moment codes that make no distinction between the type of psychotherapy or the therapy placement.
The second set of individual psychotherapy codes is add-on codes, including CPT codes 90833, 90836, and 90838. When psychotherapy is performed concurrently with an E/M service, psychiatrists must use timed add-on codes for psychotherapy to demonstrate that both services were offered, such as 90833 – 30 minutes, 90836 – 45 minutes, and 90838 – 60 minutes. A plus sign indicates add-on codes. A CPT code that explains a service always conducted in combination with the primary CPT code is known as an add-on code. For example, the CPT code +90833 is used with the primary CPT code 90832, +90836 with 90834, and +90838 with 90837. When conducted in conjunction with an evaluation and management service, use an add-on code for Personal psychotherapy, additional perspective, behaviour-modifying, and support, 60 minutes with the patient and family member (time range 53 minutes or more). As previously stated, add-on code 90838 can be used in conjunction with primary CPT code 90837.
90833, 90836, 90838 Add-on CPT Codes
- An E/M code and a psychotherapy add-on code must be important and independently identifiable to be reported.
- The type and level of E/M service are first determined by the key elements of history, investigation, and medical decision making.
- Time should not be used to decide on an E/M code.
